As the national debate over health care reform rages on, the effort to improve patient care — and control costs — is already well underway at the local level. At Atlantic General Hospital in Berlin, Md., nurses and other staffers are working to educate patients about the best ways to manage their health conditions — and avoid costly visits to the emergency room.  Bryan Russo heads over to AGH to find out who's being targeted by the program and how it may change patients' relationships with doctors.
